Do not forget to include failure to comply with any safety regulations the business should follow. In my experience, many "not-so-smart" managers still go the "no-reason" or "stupid reason" route when terminating a disgruntled employee. A reprimand notification is mostly the first step in any legal and proper worker separating procedure. If you want to revoke the agreement, I should receive a written notification within this time.
